By signing below, I, the undersigned, acknowledge and agree to the following:

Application Verification
I hereby authorize SouthPOINTE Radiator (“the Company”) to verify any and all information provided in the Dealer Application. This includes, but is not limited to, business references, credit history, financial records, and relevant licenses or permits.

Credit Check Authorization
I authorize the Company to obtain credit reports and/or perform background checks from credit bureaus or other relevant sources to assess the financial standing and integrity of the business and its principals.

Confidentiality
All information obtained through this consent will be kept confidential and used solely for the purpose of evaluating the dealer application.

Ongoing Consent
I understand that this consent remains valid throughout the course of any business relationship and may be used for periodic evaluations, provided the relationship continues.

Truthfulness of Information
I affirm that all information provided in the dealer application is true, accurate, and complete to the best of my knowledge.
